Certifix Key Requirements & Preparation
- Identification (Two Forms): Bring at least one primary photo ID (e.g., U.S. Passport, Federal Government PIV Card, Uniformed Services Identification Card, Dept. of Defense Common Access Card, or driver’s license).
- Secondary ID A second form of identification, such as a birth certificate, state ID card, or social security card, is often required.
- Documentation: Bring any required paperwork from your requesting agency, such as a completed Request for Live Scan Service Form or your ORI number.
